Applicants who wants to conduct research in the Catholic Schools in the Diocese of Ballarat must submit this form to DOBCEL along with the following documents:

•  Letter of request to principal 
• Plain Language Statement (PLS) describing the research – for principal 
• PLS – for participants (written in a style appropriate to their age) 
• PLS – for parents (if applicable) 
• Copy of all research instruments (e.g. surveys, interview questions) 
• Consent Form for participants 
• Consent Form for parents (if students are participants) 
• Notification of approval from the HREC (if available*) 
• Any additional information to support the application (optional) 

The Research Panel reviews applications during Week 8 of each term. Applicants will receive an outcome by Week 10 of the same term.

Should an application be urgent, the applicant is asked to flag this in the initial email and provide the reason/s for an out-of-session review.
